    Astia Health Clinical Services, S.C. does not replace the existing primary care physician relationship. Astia Health Clinical Services, S.C. is not an insurance product nor a prescription fulfillment warehouse.


    Astia Health Clinical Services, S.C. physicians and providers reserve the right to deny care for potential misuse of services.


    Astia Health Clinical Services, S.C. reserves the right to not prescribe DEA controlled substances, non-therapeutic drugs and certain other drugs which may be harmful because of their potential for abuse.


    Astia Health Clinical Services, S.C. does not guarantee that a prescription will be written. Astia operates subject to state regulations.

    California Online Privacy Protection Act

    Fair Information Practices


    The Fair Information Practices Principles form the backbone of privacy law in the United States and the concepts they include have played a significant role in the development of data protection laws around the globe.     Astia Health does not create or maintain a Hospital directory. Further Astia Health does not create or maintain psychotherapy notes in this practice.

    Records pertaining to substance abuse, or assessment and treatment of non-psychotherapeutic mental health issues require additional written permissions prior to any release. Astia Health will not release this information under any circumstances without your specific written authorization. This authorization is revocable by you at any time for any reason.


    Our Responsibilities


    • We are required by law to maintain the privacy and security of your protected health information.
    • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.

    • With this consent, Astia Health may mail to my home or other alternative location any items that assist the practice in carrying out treatment, payment and health care operations, such as appointment reminder cards and patient statements.
    • I hereby authorize the release of medical information to any providers involved in the direct care associated with my illness and treatment.
    • I hereby authorize the release of medical information to insurance carriers necessary to process the claim and hereby assign to Astia Health all payments for medical services rendered.  I also request that payment for government benefits go directly to the party who accepts assignment.
